WHAT IS A DREAM TEAM MEETING?
A Youth Transition Decision Making Meeting (YTDM - or a “Dream Team”) is a type of shared decision-making for youth age 16 or older who will age out or have already aged out of foster care OR for youth in the community who have been at risk and would like assistance in planning for the challenges of becoming an adult. The youth agrees to hold the meeting and works with a trained facilitator to set this meeting up. Everyone who cares about the youth gets together to help solve problems and to develop a plan that supports the youth in moving forward as they approach adulthood. The youth will receive a copy of the plan and any others identified by the youth in order to help with moving forward with the plan developed.
HOW IS THIS DIFFERENT FROM DISCUSSING THINGS AT HOME?
In a Dream Team meeting, a neutral person, a facilitator, helps everyone involved to focus, brain storm and problem solve in a creative and organized way. The youth can also invite friends, family members, and professionals like teachers, coaches, employers, counselors, mentors, who play Important roles in their lives. The meeting will focus on the youth’s strengths and concerns noted in the areas of education, housing, health, and relationships as they approach adulthood.
WHAT ARE THE BENEFITS?
Dream Teams provide an opportunity for youth age 16 and over with their supports to come together and agree on what’s best for their future and at NO cost to the youth. This is a voluntary program offered to this age group. These meetings can include any person the youth feels plays an important part in the their life as they move forward. As a result, a comprehensive plan for the youth to meet the Identified goals for the future are established.
